• Billing Address

• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• First 6 and Last 4 digits of the Credit you are being charged on
• Date of Last Charge
• Amount of Last Charge

2. Tell the representative that you’d like to cancel
3. Make sure that you won’t be charged again by asking the rep explicitly
4. Be sure to get a confirmation number or email for the cancellation

3. Hit the link that says “My Account”
4. Hit the link that says “Edit Meal Plan”
5. Now put in your info where it asks for it
6. Retain any confirmation numbers or emails you receive for your records

4. Request cancellation in the message bo
