• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge
• Account Number
• Date of Birth

Follow these steps:

1. Using your phone, call the phone number of the gym you signed up with
2. Ask for and write down the steps you need to take to cancel
3. Give the rep the info they need to find your account and ask if you’re in a contract that would prevent you from cancelling

5. Be sure to get a confirmation number or email for the cancellation as proof that you cancelled
6. Retain any confirmation numbers or emails you receive for your records
